PeaceHealth is seeking a RN – Home Health & Hospice for a 0.50 FTE, On Call, after hours / Night position. Hourly compensation is $39.21 - $74.43; hiring rate dependent on qualifications, professional experience, work location, and/or terms of Collective Bargaining Agreement.

Get paid to make yourself available for Home Health and Hospice after hours calls! The candidate is not required to work unless they receive a call. Guaranteed to be paid 20 hours per week! Benefits eligible!

The scheduled rotation for this position is 2 days on 2 days off, 5 days on, 5 days off. The hours of coverage are from 4:30pm – 8:00am. The candidate is not required to work unless a call is received.

This position is responsible for providing direct skilled nursing care and coordination/management of patients admitted to hospice/clinic services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation.

DETAILS OF THE POSITION:

  1. Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services.
  2. Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.
  3. Performs physical assessments, develops, coordinate, modifies and manages written individualized plan of care, as ordered by the primary physician, to meet patient’s long and short-term medical goals and appropriate/timely home hospice outcomes.
  4. Develops and clinically manages the Home Health Aide’s care plan according to the patient’s evolving personal are needs.
  5. Coordinates care, collaborates and communicates with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.

WHAT YOU BRING:

  • Graduate of an accredited school of nursing required. BSN preferred.
  • Minimum of one year of home health or hospice/end of life nursing experience required; hospice experience preferred.
  • Current Registered Nurse licensure in the state in which nurse will be practicing.
  • Current BLS (Basic Life Support) required.
  • Valid driver’s license (OR, WA or AK as applicable) and proof of auto insurance required.
  • Able to Provide patient care in an uncontrolled home environment; operates motor vehicle on urban and rural roadways up to 25% of each day.

This position is represented by a collective bargaining agreement. There may be more than one opening on this posting.
