PeaceHealth is seeking a RN Home Care Case Manager for a Full Time, 1.00 FTE, Day position. Hourly compensation is $39.21 – $74.43; hiring rate dependent on qualifications and professional experience and/or terms of Collective Bargaining Agreement.

Responsible for providing direct skilled nursing care through the coordination/management of patients admitted to home care utilizing the professional healthcare process of assessment and plan of care development to achieve the desired patient outcomes.
