PeaceHealth is seeking a Clinical Liaison Assistant - Home Health for a Part Time, 0.80 FTE, Day position. Hourly compensation is $22.15 – $33.22; hiring rate dependent on qualifications, professional experience, and/or work location.

Provides excellent customer service, diverse clerical and/or administrative support in a variety of settings; work may include but is not limited to planning, organizing, and coordinating communications, presentations, spreadsheets, project management, data, and records management, etc. as defined within the scope of the role. Provides direct assistance to the clinical liaisons while facilitating appropriate referral discharges to Home and Community Services.

DETAILS OF THE POSITION:

  1. Functions as a key decision maker, utilizing specific knowledge related to each patient for Home and Community admission planning purposes. Plans for potential changes in scheduling capacity. Monitors for and anticipates acute care discharge planning changes, communicating, coordinating, and securing start of care times to Home and Community services.
  2. Applies knowledge and independent judgment while following established policies and procedures to complete a wide range of assignments and resolve problems related to the liaison workflow. Provides accurate tracking and follow up of referrals while providing excellent communication and customer service to patients, families, and referral sources.
  3. Provides guidance and training to other staff and serves as a backup to intake. Ensures growth of Home and Community care services inside and outside of acute care hospitals.
  4. Uses problem solving skills when unexpected needs arise. Utilizes provided resources to keep the Home and Community admission process progressing. Uses critical thinking to understand limitations related to processing referrals and acts on that understanding to release appointment times to other patients when necessary or keep the secured appointment time while working out final details of pending referrals.
  5. Applies knowledge of Home and Community services, discharge planning, and exceptional customer services skills and aptitude. Communicates with the nurse liaison team for clinical needs and identifies when to request nurse assistance, including but not limited to needing orders for treatments, unexpected acuity assessment, clinical questions from referral source or patients and documents necessary to meet conditions of participation.

WHAT YOU BRING:

  • High School Diploma or equivalent preferred
  • Minimum of three years of progressively responsible applicable experience in a professional office.
  • Minimum of one year of customer service experience.
  • Proficient in Windows based PC applications (including Word, Excel, PowerPoint, and Outlook).
  • Must be fully vaccinated for COVID-19 including 2 doses of a 2-dose series or 1 dose of a 1-dose series plus 14 days beyond the final dose prior to start date.
