PeaceHealth is seeking a RN Wound and Ostomy for a Relief, 0.000001 FTE, Day position. 

Hourly compensation is $48.22 – $75.72; hiring rate dependent on qualifications, professional experience, work location, and/or terms of the Collective Bargaining Agreement.

Provide assessment, formulates a plan of care and acts as a resource to patients, staff members and physicians in providing acute and rehabilitative care specializing in the needs of wound and ostomy patients following the nursing process and established standards. Participate as a member of the healthcare team by communicating and documenting pertinent patient information and providing patient and family teaching.

Details of the Position

1. Provide assessment and planning to include consultation and direct assistance to physicians and staff in the care of patients with draining wounds and fistulas; complicated drain tubes, altered skin integrity due to drainage, incontinence or pressure. Assess bio-psycho-social status, economical, environmental, cultural, spiritual, community and other factors affecting the patient’s return to optimal health. Establish and maintain a plan of care in collaboration with patient, family and health team members including individualized interventions, goals, outcomes and education and modify or continue plan based on the patient response.
2. Provide pre-operative counseling, intervention and evaluation to include direct communication with patients scheduled for urinary or fecal diversion surgery, implementing nursing and medical interventions using appropriate resources, skills and knowledge. Make appropriate referrals and provide patient and family teaching to facilitate desired patient outcomes. Evaluate effects of care provided and identify changes in patient condition and patient care and education post-operatively to assist patients to return to self-care and psycho/social rehabilitation.
3. Perform effective communication and documentation by communicating and documenting interventions, patient responses according to standards. Utilize recognized organization communication standard. Keep abreast of and comply with departmental and organizational changes and events through reading appropriate communications and attending unit and assigned meetings.
4. Delegate only those tasks that are appropriate within the scope of practice of other healthcare providers. Retains the responsibility and accountability for the nursing care of the patient.
5. Provide patient, family and staff (physicians, RN's, etc.) education to include identifying patient and family teaching needs. Involve appropriate family members in teaching. Utilize available education resources. Provide inservices/education relating to ostomy, wound, skin and incontinence care.
6. Contribute to and participate in ongoing quality improvement programs designed to monitor and evaluate the quality and appropriateness of patient care or services provided by pursuing opportunities to improve. Establish new policies and procedures, review/revise new or current policies and procedures within the realm of Wound Ostomy expertise.
7. Perform medication administration to include consistently adhering to medication administration policies and procedures. Demonstrate understanding of effects/side effects and combination effects of medications. Resolve medication discrepancies as they occur.
8. Perform general duties to include maintaining a working knowledge of infection control, risk management and emergency preparedness plans. Actively participate in performance improvement initiatives. Demonstrate knowledge of hospital, interdepartmental and unit goals, standards and operations. Maintain and update knowledge of and is responsive to the laws and regulations which affect his/her practice.
9. Perform other related duties as may be assigned.

What You Bring

• Graduate of an accredited school/college of nursing.
• Completion of a Wound Ostomy Continence Nursing (WOCN) Accredited Program required.
• Minimum three years recent acute care RN (or equivalent education and/or experience).
• Inpatient/Outpatient wound and ostomy nursing experience preferred.
• WA State RN Licensure.
• Basic Life Support (BLS) for Healthcare Providers required upon hire.
• Certified Wound Ostomy Continence (CWOCN) or Certified Wound Ostomy Nurse (CWON) certification within one year of hire.
