PeaceHealth is seeking a LPN Home & Community for a Part Time, 0.60 FTE, Day position. The general salary range for this job opening at PeaceHealth is $27.47 – $41.21. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc.

Hiring bonus and relocation assistance may be available.

Under the supervision of an RN, the LPN provides direct nursing care, within the scope of their license, to patients as directed by the physicians and other healthcare providers. Cooperative decision making is expected in the execution of duties.

Details of the position:

  • Implements and evaluates patient care under the supervision of an RN. Participates in the development of a patient-specific care plan.
  • Provides direct patient care in patient homes, including administering medications and treatments as prescribed. Understands and applies knowledge in edema/lymphedema management, venipuncture, management of PICC lines, implanted ports, post-surgical drains, and pulmonary and abdominal drains.
  • Provides focused systems and wound assessment. Reports acute medical problems and condition changes to the case manager and/or attending physician, as directed.
  • Demonstrates proficiency in communicating effectively with individuals diagnosed with mental and behavioral disorders and operates efficiently in an independent home setting.
  • Documents patient care in health record according to department standards.

What you Bring:

  • Graduate of an accredited State Board of Nursing LPN program.
  • A minimum of one-year clinical experience as an LPN preferred.
  • Current licensure as an LPN in the state in which the nurse will be practicing.
  • Current BLS (Basic Life Support) certification required at date of hire.
