Description
PeaceHealth is seeking a RN Clinical Liaison - Hospice for a Part Time, 0.80 FTE, Day position. The salary range for this job opening at PeaceHealth is $45.30 – $71.12. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
Hiring bonus and relocation assistance may be available.
Provides an interface between health care providers and PeaceHealth Hospice. Develops and implements a program to contact all potential referral sources within the hospital. Responsible for establishing, growing, and maintaining referral volume and relationships with hospital staff. Communicates with existing referral sources and identifies new opportunities within the hospital. Delivers exceptional customer service, and assists with advanced care planning discussions, informational Hospice consults, Hospice education to serve patients and families.

WHAT YOU BRING:
Must be fully vaccinated for COVID-19 including 2 doses of a 2-dose series or 1 dose of a 1-dose series plus 14 days beyond the final dose prior to start date.
