Location: Florence, Oregon
Pay Range: $47.40 – $74.48 per hour (Rate based on experience, education, seniority, and collective bargaining agreement)

PeaceHealth is seeking a Registered Nurse – Hospice to join our dedicated team in a part-time day shift role, providing skilled and compassionate care to patients and families navigating end-of-life care at home.
About the Role:
As a Hospice RN, you’ll deliver skilled nursing care and manage individualized care plans to support patients’ medical and emotional needs. You’ll work closely with an interdisciplinary team, offering direct care, education, and support to patients and caregivers throughout their hospice journey.
Key Responsibilities:
Evaluate patients and home environments to determine eligibility and safety for hospice care
Deliver direct skilled nursing care and caregiver instruction per the plan of treatment
Develop and manage individualized care plans in coordination with physicians and care team
Coordinate services across the care continuum and participate in discharge and bereavement planning
Ensure timely and accurate clinical documentation
Qualifications:
Graduate of an accredited nursing program (BSN preferred)
Active RN license in Oregon
2 years of acute, clinical, or hospice/home health experience
Current BLS certification
Valid driver's license and proof of insurance
Strong assessment, coordination, and communication skills
