LPN – Home & Community Hospice
Part-Time, 0.80 FTE – Days | PeaceHealth | Florence, Oregon
Hourly Rate: $28.02 – $42.04 (dependent on experience, education, union agreement, and other factors)

Are you a compassionate LPN who thrives in a more personal, independent care environment? PeaceHealth is seeking a Licensed Practical Nurse to join our Home & Community Hospice team in beautiful Florence, Oregon.
In this rewarding part-time, day-shift role, you’ll provide one-on-one care in patients’ homes, supporting individuals and their families during one of life’s most meaningful transitions. Working under the supervision of an RN, you’ll administer medications and treatments, monitor symptoms, assess wounds, and educate patients and caregivers—all while promoting comfort, dignity, and peace.
What you’ll do:
Deliver direct, patient-centered care in home settings
Support patients with medication, wound care, and symptom management
Recognize and report changes in condition to the RN/case manager
Document care thoroughly and accurately
Educate patients and caregivers in managing chronic and terminal conditions
Collaborate with a supportive interdisciplinary hospice team
Qualifications:
✔ Graduation from an accredited LPN program
✔ Current Oregon LPN license, BLS certification, driver’s license, and proof of insurance
✔ 1+ year of LPN experience preferred, especially in hospice or home health
