PeaceHealth is seeking a Licensed Clinical Social Worker to work with the Behavioral Health Transition Team in Eugene, Oregon. This is team-based care in an outpatient setting.

The selected LCSW will join a team providing intervention to suicidal individuals working in an evidence-based program, CAMS. You will have a true impact by reducing barriers to care for clients while working with them for up to six months. The team members have long-standing longevity and collegiality.
Coordinates and oversees the plan of care and service delivery of behavioral health consumers across the continuum of illness; promotes effective utilization and monitoring of health care resources; and works collaboratively with the interdisciplinary team to achieve optimal clinical outcomes. Provide services to consumers with commercial insurance and Medicaid and Medicare.
