Job Description


PeaceHealth is seeking a full-time RN Navigator to coordinate the Blood Management Program at Southwest Medical Center in Vancouver, Washington. This position is in the Anesthesia Pre-Op department and is a 1.0 FTE, Day shift position, working 8-hour shifts.    Hourly compensation starts at $42.87, more depending on experience.


Responsible for the coordination of Patient Blood Management patients. In collaboration with the Medical Director(s), referring physician and interdisciplinary team(s), the Patient Navigator coordinates disease specific multidisciplinary conference; subsequent specialty consultations; and provides care as needed for patients and their families utilizing resources efficiently and effectively.

Coordinates the development and performance of Patient Blood Management educational activities, program evaluation and outcomes research.


  • Act as a single point of contact for patients and families including telephone triage regarding symptom management. Assist patients and families in understanding their diagnosis, treatment options, and the resources available, by utilizing excellent Interpersonal skills.
  • Develop and maintains clinical guidelines, policies, protocols and documentation for practice in patient blood management.
  • Develop patient education program, tools, and documentation. Reinforce patient education and direct patients and families to available resources and supportive services. Collaborates with nursing education regarding competencies
  • Ensure patients receive a treatment plan that is understandable and feasible and coordinate timely scheduling of tests, procedures, specialty appointments and treatments, including initiating appropriate referrals (both internal and external) to community resources. Follow patients through the continuum of care, in collaboration with key stakeholders in the treatment plans.
  • Assess for clinical, emotional, spiritual, psychosocial, financial and other patient/family needs.
  • Coordinate multi-disciplinary planning conferences and education; develop concise patient summaries for use by the care team and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines. Collaborate and follow-up with the multidisciplinary patient blood management team.
  • Along with leadership guides appropriate Patient Blood Management program growth that is in accordance of governing bodies and best practice.
  • Facilitate quality improvement initiatives, assist with the development of PBM metrics.
  • Performs other duties as assigned.



  • Graduate of an accredited school of nursing. BSN or Master’s degree preferred.


  • Minimum of five years experience in nursing including two years of acute hospital care as appropriate to work setting
  • Management and/or program development experience desirable.
  • Must be able to work in a self-directed environment..


  • Current Registered Nurse licensure in the state in which nurse will be practicing
  • BLS (Basic Life Support) certification required.


  • Effective written and verbal communication skills
  • Proficient computer skills including MS Office applications
  • Insight into the roles and responsibilities of the interdisciplinary team members
  • Ability to identify and collect quality data and provide interpretation

  • Work requires frequent exposure to minor cold, heat, poor ventilation or sharp instruments. Reasonably anticipated exposure to blood and body fluids once per month or more.
  • Duties require the ability to concentrate and pay close attention to detail (more than 65% of work time).
