The Ideal Candidate

  • Nurse Practitioner or Physician Assistant with post-certification experience and/or intentional training focus in Palliative and Hospice Care, Gerontology
  • Excellent communication skills, including being comfortable having Critical Illness conversations with patients and families
  • Experience working with an Interdisciplinary team
  • Collaborative mindset, particularly with Providers and referral partners
  • Familiarity/experience with pain management, medication conversions; comfortable with prescribing skills
  • Familiarity/experience with Advanced Care Planning

  • Innovative team and program
  • Our Palliative Care and Hospice team offers care, support, guidance, and reflection in medical, emotional, and spiritual areas, with a focus on preventing and alleviating suffering and improving a patient’s quality of life
  • This position is primarily Inpatient Palliative Care with some outpatient work; 100% consults; no primary care; work collaboratively with inter-disciplinary team to prioritize and to address urgent consult requests; Hospitalist team supports admitting needs
  • This APC will be part of larger Palliative Care and Hospice Provider Team, with cross-coverage and cross-training provided
  • Epic EMR
