PeaceHealth is seeking an exceptional full-time Nurse Practitioner to join a vibrant multi-disciplinary Hospice Care team in Longview and Vancouver, Washington.

At PeaceHealth, Our Vision is that every person receives safe, compassionate care; every time, every touch. The PeaceHealth Hospice team is dedicated to easing the fears, anxieties, and discomforts that occur during the advanced stages of illness. The Hospice Care program provides a full continuum of care for patients and their families. Medical, spiritual, and emotional assistance helps patients and their families to ease the symptoms of life-threatening illnesses. This opportunity will require traveling throughout Cowlitz and Cowlitz Counties, but this position will be primary based out of St. John Medical Center in Longview, Wa. Candidates with Hospice-Palliative care experience and a certification in Hospice & Palliative Care are highly desired.

• On-site complex case management team, social workers, chaplain, and including a partnership with the Ray Hickey Hospice House

• Schedule: Monday - Friday, with rotating weekend with call once monthly
• Anticipated patient load: ~2-6 face to face or Telemedicine visits daily
