Seeking an Advanced Practice Clinician who would be interested in this new opportunity as a Program Coordinator for our new Lung Nodule/Cancer Screening Program at PeaceHealth Southwest Medical Center in Vancouver, Washington.  The position is 50% clinical and 50% administrative.

The selected Nurse Practitioner or Physician Assistant will be responsible for the coordination of the Lung Nodule and Lung Cancer Screening Program, including the insuring following duties: 

  • Ensuring continuity of care through optimized patient assessment, clinical triage coordination, direct communication with physicians and daily patient management. 
  • Providing a continuum of patient and family education from referral to follow-up.
  • Ensuring patients receive a treatment plan that is understandable and feasible, while coordinating timely scheduling of tests, procedures, and appointments. 
  • Serving as the primary contact for referring physicians, patients, families, hospital and office staff, and vendors. 
  • Developing case review presentation for case review meetings.
  • Working closely with data analyst to collect registry data, track outcomes and support strategic planning processes. 
  • Participating in quality improvement initiatives. Role model clinical expertise by educating community and hospital staff.
  • Formulating appropriate differential diagnosis and identification of patient needs. Coordinating and preforming appropriate testing modalities, to diagnose and direct plan of care to appropriate path for patient condition.

EDUCATION:

  • Must fulfill educational requirements to obtain ARNP or PA license in Washington state. 

EXPERIENCE/TRAINING:

  • Advanced knowledge related to pulmonary disease management in adults required. 
  • Management and/or program development experience preferred.
  • Must be able to work in self-direct environment. 

LICENSE/CERTIFICATION: 

  • Current Nurse Practitioner or Physician Assistant license in state of practice required or ability to obtain prior to start date. 
  • Current BLS (Basic Life Support) certification required at date of hire. 
  • Current ACLS (Advanced Cardiac Life Support) certification required at date of hire.
