Seeking an Advanced Practice Clinician to work in our new Lung Nodule Program within the Pulmonary Department at PeaceHealth Southwest Medical Center in Vancouver, Washington (Portland metro area).

The selected Nurse Practitioner or Physician Assistant will be responsible for: 

  • Ensuring continuity of care through optimized patient assessment, clinical triage coordination, direct communication with physicians and daily patient management. 
  • Ensuring patients receive a treatment plan that is understandable and feasible, while coordinating timely scheduling of tests, procedures, and appointments. 
  • Providing a continuum of patient and family education from referral to follow-up.
  • Serving as the primary contact for referring physicians, patients, families, hospital and office staff, and vendors. 
  • Developing case review presentation for case review meetings.
  • Working closely with data analyst to collect registry data, track outcomes and support strategic planning processes. 
  • Participating in quality improvement initiatives. 
  • Role model clinical expertise by educating community and hospital staff.
  • Formulating appropriate differential diagnosis and identification of patient needs. Coordinating and preforming appropriate testing modalities, to diagnose and direct plan of care to appropriate path for patient condition.

QUALIFICATIONS:

  • Nurse Practitioner or Physician Assistant license in Washington state or ability to obtain prior to start date. 
  • Advanced knowledge related to pulmonary disease management in adults required. 
  • Management and/or program development experience preferred.
  • Current BLS (Basic Life Support) certification required at date of hire.
  • Current ACLS (Advanced Cardiac Life Support) certification required at date of hire.
