PeaceHealth is seeking candidates for a Manager role over  Medical Social Work/Utilization Review at Southwest Medical Center in Vancouver, Washington.  Join the Care Management team in this  Full Time, exempt, day shift position.

JOB SUMMARY

Responsible for developing, organizing, implementing, and evaluating the provision of Social Work Care Management services. Ensures that program practices are in accordance with accepted standards and aims for the program to be a model of best practices. Coordinates department activities so that hospital and community resources are appropriately and effectively utilized on behalf of patients and their families. Strives to optimize continuity of care and seamless transition of care.

ESSENTIAL FUNCTIONS

  • Provides direction, leadership, and management oversight of the department, including setting standards of practice.
  • Participates in strategic and operational planning, implementation and evaluation to ensure effective patient Care Management and Utilization Management.
  • Works with physicians, nurse care coordinators, and nursing unit managers to ensure effective care coordination.
  • Supervise staff and give technical direction, monitor and prioritize workflow.
  • Manage the selection, training and development of staff.
  • Develops and monitor department budget. Oversees operational planning process, e.g. identifying goals, measures and targets.
  • Facilitates internal and external department communication.
  • Ensure adequate staffing and effective utilization of resources.
  • Assure services are in compliance with professional standards, state and federal regulatory requirements.
  • Actively works to continuously improve all services and promote work efficiencies.
  • Performs other duties as assigned.

QUALIFICATIONS:

EDUCATION:

  • Master’s degree or PhD in Social Work from an accredited school of Social Work required.

EXPERIENCE/TRAINING:

LICENSE / CERTIFICATION:

  • WA State Licensed Clinical Social Worker (LCSW) and/or by the Academy of Certified Clinical Social Workers (ACSW) preferred.

KNOWLEDGE/SKILLS/ ABILITIES:

  • Strong communication skills, both verbal and written.
  • Strong group facilitation, team building and project management skills.
  • Strong leadership and decision-making skills and ability to mentor/coach, guide, educate and motivate others.
  • Proficient computer skills including MS Office applications.
