Ideal Candidate

• Board Certified Nurse Practitioner with at least two years of post-certification experience in primary care
• Interest in providing geriatric care in community nursing homes as well as in the outpatient primary care setting
• Experience and interest in providing standard primary care visits, ambulatory geriatrics consultations, palliative care, clinical social work services, management of the medication therapy program, and anticoagulation therapy
• Possess a passion for serving geriatric patients, and dedication to providing the highest quality care for every patient

Position Highlights

• The Center for Senior Health (CSH) is an outpatient geriatrics clinic that provides comprehensive ambulatory care for adults age 65 and over
• CHS cares for an average of 2,000 patients per month
• Collegial, dedicated, and supportive team
• Full-service laboratory and imaging onsite
• Clinic Hours are Monday through Friday 8:00 am – 5:00 pm
• Epic EMR

• Guaranteed first-year income, based on years of direct experience (0-3, 3-5, 5-9, 10+): $121,862.00-$144,913.00
• Employment Bonus
• Relocation stipend based on miles between locations

• CME: up to $3,300 per calendar year for CME and/or professional dues/fees (based on 1.0 FTE; prorated for lower FTE)
