PeaceHealth is seeking a Community Health Worker (Post Acute) for a Full Time, 1.00 FTE, Day position. Hourly compensation starts at $21.30, more depending on experience.

Must be fully vaccinated for COVID-19 including 2 doses of a 2-dose series or 1 dose of a 1-dose series plus 14 days beyond the final dose prior to start date.

JOB SUMMARY 

Responsible for trying to increase access, reduce risk and improve quality to healthcare and social services for CIN patients who are receiving post acute care. The goal is to partner with patients in providing safe and seamless transitions across the care continuum. Providing patient support by coordinating and finding stable, consistent health and community services. 

ESSENTIAL FUNCTIONS 

  1. Ensure patient has primary care services. 
  2. Coaches patients in how to effectively use a medical home. 
  3. Reduces cultural and socio-economic barriers between patients and health providers. 
  4. Monitors and follows-up on service plans with clients via telephone and home visits. 
  5. Educates patients about self-care and habits that can impact health. 
  6. Coordinates patient transportation and accompaniment as needed to scheduled appointments. 
  7. Assures patients receive appropriate and timely services by making referrals and providing health and community resources. 
  8. Conducts in home needs assessment and assists in developing care/action plans addressing social determinints of health. 
  9. Documents, tracks and reports program data and statistics. 
  10. Conducts outreach and collaboration with community social service agencies and health providers in order to improve patient access. 
  11. Coordinates patients care with post acute care team, facilities/ agencies and other medical personnel. 
  12. Facilitates advance care planning conversations and completion of documents. 
  13. Performs other duties as assigned. 

QUALIFICATIONS(required unless otherwise noted) 

EDUCATION: 

  • Minimum of 2 years years relevant post-secondary education or training preferred. 

EXPERIENCE/TRAINING: 

  • Minimum of two years experience working in human services. 
  • Experience working with healthcare systems and multi-cultural communities preferred. 

LICENSE/CERTIFICATION: 

  • Valid driver’s license, personal vehicle and safe driving record. 
  • Advance Care Facilitator Certification (required within 1 year of hire) 
  • Notary licensure/certification (required within 1 year of hire) 

OTHER SKILLS: 

  • Proficient computer skills including MS Office applications. 
  • Excellent communication skills, both verbal and written. Ability to motivate others. 
  • Demonstrated knowledge of an ability to apply age specific principles of growth and development and life stages to meet each patient’s needs. 
  • Ability to work with people from diverse backgrounds and experiences. 
  • Ability to understand and communicate the concepts of the interrelatedness of body, mind and spirit to health and wellness. 
  • Ability to promote safe aging in place by leveraging knowledge and involvement of post acute services and resources. 

PHYSICAL/COGNITIVE REQUIREMENTS

  • Work requires fairly light physical exertion from 34 to 65% of the time.
  • Ability to lift objects weighing 30 lbs. or less.
  • Work is performed under normal working conditions with adequate lighting and ventilation.
  • Job duties frequently require intense concentration and attention to detail (34-65% of work time).
