PeaceHealth Cottage Grove is seeking a Occupational Therapist for a Part Time, 0.60 FTE, Day position. Hourly compensation is $39.24 - $58.86; hiring rate dependent on qualifications, professional experience, and/or work location.

Cottage Grove is a community of approximately 9,000 people located in the beautiful Willamette Valley, along the Row River, 20 miles south of Eugene and the University of Oregon and serves a population of approximately 25,000 people in the surrounding area. It boasts a modern high school and is the recipient of a national "All-American City Award". Cottage Grove is one of only two cities in the State of Oregon to have received the award twice. No other city in Oregon has received the award more than twice. It is also known as the “Covered Bridge Capital of Oregon” six of the covered bridges in Lane County are located in or around the City of Cottage Grove. The community boasts in many outdoor activities, such as hiking, fishing, biking, golfing, boating, gardening. There are six waterfall hikes within 30 miles, Row River trail (16 miles long), there is a local live theater and a downtown historic district.

Annual community events include Historic Home Tours, the Cottage Grove Rodeo, Bohemia Mining Days and the Family Fun Fair-Western Oregon Exposition.

JOB SUMMARY

Responsible for the evaluation of patients, determining plan of care and goals of treatment, participating in treatment of patients participating in treatment of patients within the scope of Occupational Therapy. May provide oversight to therapy assistants and/or students in following the plan-of-care and treatment of patients appropriate to the age of the patient served; may provide supervision to therapy aides and/or volunteers. Works collaboratively as a part of a patient care team toward the best outcomes for patients.

What you will do:

  1. Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation and team members.
  2. Completes patient treatment according to plan of care and modifies plan to meet patient needs, and updates plan as appropriate.
  3. Completes documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards.
  4. Collaborates with other healthcare professionals as necessary to ensure smooth continuous care of the patients. May act as a liaison with nursing, medical and other therapy staffs to facilitate problem solving and coordination of other services and act as an educational resource.
  5. May mentor and provide oversight to new caregivers, occupational therapy assistants, and/or therapy aides. May provide clinical instruction and training to students.

What you bring:

  • Bachelors’, Masters’ or Doctorate degree from an Occupational Therapy program accredited by the Accreditation Council for Occupational Therapy Education required.
  • Current state of Oregon license as an Occupational Therapist required.
  • Current BLS (Basic Life Support) certification through American Heart Association required at date of hire.

  • Strong organizational and analytical skills
  • Ability to manage multiple priorities, projects and display flexibility in a fast paced and changing work environment
  • Ability to assess patients’ conditions and exercise independent judgment

REGIONAL/LOCATION SPECIFIC NOTES

Oregon West Network – Cottage Grove: May function in activities coordinator role in addition to Occupational therapy duties. Driver’s license preferred to allow caregiver to perform home visits as needed. For caregiver with valid driver’s license, documentation of both current driver’s license and auto insurance is required.

COTTAGE GROVE PHYSICAL/COGNITIVE REQUIREMENTS:

  • Work requires moderate physical exertion up to 33% of the time.
  • Ability to lift objects weighing 50 lbs. or less.
  • Work requires frequent exposure to minor cold, heat, poor ventilation or sharp instruments. Reasonably anticipated exposure to blood and body fluids once per month or more.
  • Job duties frequently require intense concentration and attention to detail (34-65% of work time).
  • Provides patient care in an uncontrolled home environment; operates motor vehicle on urban and rural roadways up to 25% of each day.
