PeaceHealth is seeking a Clinical Social Worker - Care Management for a Full Time, 1.00 FTE, Day position partially remote.

Job Summary

The Clinical Social Worker coordinates the development and implementation of a patient care/treatment plan through use of diagnosis and clinical assessment of psychiatric, chemical dependency and psychosocial needs as they relate to medical and behavioral care needs. Provides disposition and discharge planning, risk assessment, case management, and coordination of services to provide continuity of care

Details of the position

  1. Completes appropriate intake documentation.
  2. Provides ongoing assessment.
  3. Develops treatment plan in collaboration with the multi-disciplinary treatment team. 
  4. Manages multiple complicated cases.
  5. Conducts therapeutic sessions with patients and/or families, as well as community staffing, in coordination with the physician and treatment team.
  6. Performs other duties as assigned.

What you bring

  • Master’s Degree in Social Work from an accredited graduate school of social work.
  • Must have LCSW or must have CSWA (Clinical Social Work Associate) status.
  • Valid Oregon driver’s license required.
  • Basic Life Support (BLS) certification required within 60 days of hire.
  • Must attend PROACT (crisis management) training provided by hospital.
  • Knowledge of community resources, knowledge of Medicare, Medicaid and insurance requirements as these relate to continuing care.
  • Two or more years of clinical experience with designated population; hospital, emergency room, chemical dependency, or community agency experience preferred.

Community/Location Specific Notes


  • Master’s Degree in Social.
  • Demonstrated knowledge of an ability to apply age specific principles of growth and development and life stages to meet each patient’s needs.
  • Demonstrated proficiency in social work practice.
  • LASW, LSWAA, and CSWA credential will also meet licensure/certification requirement.
