PeaceHealth is seeking a Clinical Pharmacy Specialist - PeaceHealth Medical Group for a Full Time, 1.00 FTE, Day position. Hourly compensation starts at $62.14, more depending on experience.

Must be fully vaccinated for COVID-19 including 2 doses of a 2-dose series or 1 dose of a 1-dose series plus 14 days beyond the final dose prior to start date.

Under the direction of the Director of Pharmacy Services, this position is responsible managing the pharmacotherapy of complex patients.

ESSENTIAL FUNCTIONS

  1. Provide consultative services to medical, nursing, and pharmacy staff as well as to patients.
  2. Ensure appropriate drug therapy for patients cared for.
  3. Implement and monitor clinical protocols and guidelines and enhancing the delivery of evidence-based medicine.
  4. Provide drug-therapy related education and training to team members and/or patients.
  5. Serve on/provides consultation to hospital and/or clinic committees as requested/is appropriate.
  6. Collaborate on research initiatives.
  7. Document clinical activities and outcomes.
  8. Provides prescribed medications and compounds according to professional standards and practices.
  9. Participate in quality initiatives to enhance pharmaceutical care.
  10. Detects, monitors and reports adverse drug events and medication errors
  11. Oversees orientation, training and mentors pharmacy students, pharmacy practice residents, and pharmacy technicians
  12. May participate as a responding member of an Emergency Response/Code Blue Team
  13. Performs other duties as assigned.

QUALIFICATIONS

EDUCATION:

  • Graduate of an ACPE-accredited School of Pharmacy with a Bachelor of Science degree in Pharmacy.
  • Pharm.D or BSc Pharmacy with ACCP recognized Board Certification or successful completion of a Post-Graduate Year Two (PGY2) residency or fellowship preferred.

EXPERIENCE/TRAINING:

  • Minimum of three years of advanced pharmacotherapy specialist practice in a clinical pharmacy setting with primary focus on direct patient care, medication management, consulting and teaching preferred. Comprehensive knowledge of evidence-based clinical pharmacy practice in intended area(s) of specialty, including related equipment, techniques, practices and procedures.
  • Minimum of one year of pharmacy experience required.

LICENSE/CERTIFICATION:

  • Current licensed registered Pharmacist/Clinical Pharmacist in the state in which you will be working.
  • ACCP-recognized Board Certification or PGY2 residency preferred.
  • Licensed as a pharmacy preceptor by the state.

OTHER SKILLS:

  • Proficient use of computers including MS Office applications. Experience with computerized pharmacy systems.
  • Excellent communication skills both verbal and written. Excellent presentation skills.
  • Ability to work collaboratively as a member of an interdisciplinary team, and establish and maintain effective working relationships with other healthcare professionals, practitioners, patients and families
  • Ability to effectively support, coach, educate, mentor and lead staff
  • Strong leadership and organizational skills

REGION/LOCATION SPECIFIC NOTES

Clinical Pharmacy Specialist (Specialty Pharmacy Specialist):

  • Helps advance quality care by actively assuring the provision of safe, effective, and cost-effective medication therapy for all specialty clinic patients receiving medication therapy.
  • Works in the specialty clinic and collaborates with all patient care team members and provides at-the-elbow support for prescribers.
  • Provides face-to-face and telehealth consults with patients receiving specialty medications to identify, resolve and prevent medication-related problems as well as assess medication therapy for appropriate indication, effectiveness, safety and adherence issues.
  • Utilizes integrated and routine communication with all healthcare professionals with an emphasis placed on monitoring and documentation of specific medication therapy outcomes.
  • Interfaces with the pharmacy technician liaison to ensure medication access, adherence and patient assistance assessments have been completed and plans implemented.

PHMG Physical / Cognitive Requirements:

  • Work requires fairly light physical exertion up to 33% of the time.
  • Ability to lift objects weighing 30 lbs. or less.
  • Work is performed under normal working conditions with adequate lighting and ventilation. Reasonably anticipated exposure to blood and body fluids.
  • Duties require the ability to concentrate and pay close attention to detail (more than 65% of work time).
